Position: Client Service Coordinator

We are seeking a dedicated Client Service Coordinator to assist us with our home care clients. You will be responsible for consulting with client and their families to determine their needs, assist in developing care plans, coordinating services identified in the client’s care plan and working with the care team to evaluate interventions and the services provided. The successful applicant will display a compassionate nature, be knowledgeable about home care practices, and possess exceptional customer service skills. In addition, the preferred candidate must have caregiving experience, as direct care is also a part of the position. Competitive salary and benefits.

Requirements:

• Minimum of 4 years home care experience with 2 years direct care experience

• Excellent personal care skills.

• Strong communication skills.

• Strong personal relationship skills.

• High school graduate, BS preferred.
