Care Coordinator (Full-Time) (Care Alliance of Western Massachusetts) engages in activities designed toward eliminating barriers and improving the conditions and well-being and cultural needs of enrollees. The Care Coordinator is an integral component of the community care and long-term care management system for enrollees. They engage in activities, which incorporate a variety of disciplines and funding sources. The Care Coordinator is directly involved in the processes of outreach to assigned enrollees, assessment, long term services and supports care planning and coordination, care transitions, health and wellness coaching, care team participation, connecting enrollee's with social services and community resources, advocacy, problem solving, and gatekeeping. Bachelor's Degree in social work, human services, nursing, psychology, sociology, or a related field from an accredited college. Bi-lingual preferred.
Options Counseling (Full-Time) is an interactive, short-term decision-support process to assist consumers, family members, caregivers and/or significant others to make an informed choice about long term services, supports, and setting. The Options Counselor assists seniors and individuals with disabilities in need of long term services and supports (consumers) to make informed choices of setting and services, resources to help pay for supports and services, referrals as needed and assistance in connecting with appropriate resources. Bachelor's degree and two years' relevant experience in elder related social services or health care. Bi-lingual a plus.
Case Manager, Social Work (Full-Time) is responsible for supporting the dignity and independence of older persons or those with disabilities in need of assistance by assessing their situations, identifying unmet needs, arranging for, coordinating and monitoring services to meet those needs, and advocating with other agencies on behalf of their client. Bachelor Degree in social work, human services, psychology, sociology or related field preferred.
Candidates with a Bachelor Degree in another discipline shall demonstrate experience and/or strong interest in the field of human services via previous employment, internship, volunteer activity and/or additional studies. All positions require a valid driver's license, travel and reliable transportation.
