HSC Registered Nurse Floater - (210003ZN) The Registered Nurse Floater will split their time providing nursing care at HSC Pediatric Center and HSC Home Care. Responsible for the overall management of all nursing care for assigned inpatients and home care clients. Under the direction of the Inpatient Nurse Manager, Inpatient House Supervisor and Home Care Senior Clinical Manager, the Registered Nurse Floater provides direct and indirect patient care, assessing nursing care needs as well as planning and providing nursing care interventions, preventing complications, providing patient and family education, and promoting improvement in the patients'/clients' comfort and wellness; informing the interdisciplinary team of patients'/clients' condition as appropriate.
When assigned to handle HSC Home Care cases, the Registered Nurse Floater will provide prompt response to physical and emotional needs of clients and their families in the form of home visits. The Registered Nurse Floater will also be utilized to provide Registered Nurse coverage in the inpatient units at HSC Pediatric Center. This nurse must proficiently problem solve case management, symptom management and crisis situations that arise.
The float nurse participates as a member of the Interdisciplinary Team (IDT) to plan and provide holistic care to patients and families. Nursing care reflects observations, actions and plans that are in accordance with the IDT Plan of Care. All efforts are directed towards continuous quality improvement and family centered care. May be utilized as a charge nurse, preceptor or code team member. Minimum Education Bachelor's Degree Bachelor's Degree in Nursing (Preferred)
Required Skills/Knowledge Required Experience: 1. Minimum of two (2) to three (3) years of experience working in pediatric nursing, preferably in acute care and/or home health settings. 2. Must be able to operate office equipment such as a personal computer, copier, telephone and calculator. 3. Maintain a vehicle to be used for work with the required insurance coverage. a. Ability to travel, as required. 4. Must be familiar with and adhere to Accreditation Organizations and Medicare Conditions of Participation. 5. Must be able to ambulate, position and transfer clients. 6. Familiarity with Maryland COMAR regulations for Residential Service Agency. 7. Fluency in Spanish preferred. The ideal candidate must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients/clients served in the assigned unit/home. The Registered Nurse Floater must also demonstrate knowledge of principles of growth and development over the life span, possess the ability to assess data reflective of the patients’ status and interpret the appropriate information needed to identify each patient requirement relative to his or her age specific needs.

Job Functions HSC Pediatric Center Inpatient Job Duties- 1. Provides leadership to ensure that important processes and activities are measured, assessed and improved systematically, interdepartmentally, and intradepartmental.
- 2. Participates in interdisciplinary, interdepartmental performance improvement activities.
- 3. Adheres to the General Performance Standards of Practice.
- 4. Assesses patients; determines nursing diagnosis; plans, implements and evaluates patient care upon admission and throughout hospitalization. The assessment must include biophysical, psychosocial, environmental, self-care, education and discharge planning factors incorporating data from the patient and or family when appropriate, in collaboration with the case manager.
- 5. Delivers nursing care to the patient based on the assessment of their needs.
- 6. Documents current patient status, the care given to the patient and the progress, response and outcome of his/her care and the activities of the day on the nursing care flow sheet and progress note as indicated in the electronic medical record.
- 7. Attends family meetings as assigned.
- 8. Develops and updates weekly nursing summary and goals in the electronic medical record.
- 9. Assigns patient care to nursing staff and other unit duties based on staff members competency.
- 10. Supervises the delivery of direct patient care by nursing staff.
- 11. Administers medications according to policy and monitors patients response.
- 12. Verifies physician orders accurately and acknowledges new orders in the electronic medical record.
- 13. Acts as a member of the Code Blue Team as assigned.
- 14. Uses equipment and supplies safely and effectively.
- 15. Adheres to standard precautions and infection control policies.
- 16. Anticipates safety problems and initiates action in response to equipment or information indicating risk(s) to correct, prevent and/or minimize risk(s) to the patient, employee of hospital.
- 17. Gives and receives report.
- 18. Assesses learning needs of patient and family, then provides them with information and teach them skills as necessary.
- 19. Performs hospital wide and unit specific technical skills competently and independently in accordance with policy and procedure guidelines.
- 20. Serves as resource for other nursing staff members.
Other Pediatric Center Job Duties- 1. Acts as a charge nurse and responds to patient and unit needs accordingly;

HSC Home Care Job Duties- 1. Collaborates, documents and communicate care needs with primary care physicians and other members of the health care team. Ensures physician orders are clear, safe, effective and are carried out according to policies and procedures.
- 2. Serves as a liaison between the client, physician, staff, and/or visitors to promote a positive care environment.
- 3. Offers assistance and direction to clients, care managers, discharge planners, physicians and families in a concise, culturally, linguistically and informative manner.
- 4. Safeguards the rights of the client and protects all client information in a confidential manner.
- 5. Performs Initial assessments, develop and implement care plan for clients receiving home health services.
- 6. Performs clinical assessments for in home care and environmental safety. Makes client visits as needed, identifying and prioritizing problems and nursing care needs. Coordinates client care and services.
- 7. Carries out DOH, CMS, CHAP standards that promotes compliance with regulatory guidelines.
- 8. Effectively reports client safety issues to the Senior Clinical Manager, other members of the home care team and appropriate external agencies such CFSA.
- 9. Communicates and documents client needs and condition changes to the appropriate person(s).
- 10. Acts rapidly and appropriately in emergency situations. Assumes assigned responsibilities for the emergency preparedness to support client safety and care.
- 11. Performs health/disease/care related education that is client/family centered.
- 12. Initiate and document supervisory visits to support client care management. Observes and monitors care delegated to others.
- 13. Reports suspected client abuse, neglect or exploitation.
- 14. Reviews physician, health team reports from nurses, home health aides and ancillary staff to insure adequate staffing and knowledge of client’s conditions.
- 15. Records and reports admissions, discharges, deaths and any unusual situations to clinical management.
- 16. Problems with clients are addressed, resolved as appropriate and reported to clinical management.
